Understand what you are taking—and why.

Medication reconciliation

Review your ER prescriptions alongside your usual medications, supplements and known allergies. Discuss duplicate instructions, questions and the follow-up recommended at discharge.

Treatment follow-up

Discuss response to the prescribed treatment and concerns you want the clinician to assess. Any medication change depends on your clinical evaluation.

BP, glucose and pain follow-up

Review blood pressure or glucose readings after the ER, and discuss clinically appropriate non-controlled pain management. Ongoing chronic care transitions to your regular clinician.

No controlled-medication refills.

Medication limits

Follow Up Dr. does not refill opioids/narcotics, benzodiazepines or stimulants. No prescription, refill or particular treatment is guaranteed. Bring your medication bottles or an accurate list to your visit.

Short-term transition care

We focus on recovery after discharge for up to 14 days when appropriate. Long-term medication management belongs with your primary care clinician or specialist. AM&A can be your ongoing medical home if you choose to establish care.

Unstable or having an emergency? Go back to the ER. For chest pain, severe trouble breathing, stroke symptoms, uncontrolled bleeding, or another medical emergency, call 911. Follow your ER return precautions. Do not wait for a follow-up visit or a reply to a request.

1

Request your visit

Have your discharge date, preferred visit format and insurance plan name ready. A request does not confirm an appointment.

2

Connect with our team

We review fit, availability and insurance or the fixed cash option. Then we help you complete secure booking and intake.

3

Share paperwork. Meet your clinician.

Please upload a photo or screenshot of your ER discharge paperwork through the secure intake link we provide, if available. Bring your questions and medication list. Missing paperwork is not an automatic barrier.

Before your follow-up.

Should I stop a medication before my visit?

Continue following your ER discharge instructions unless a treating clinician advises otherwise. Contact a clinician promptly about medication concerns. For severe symptoms or a suspected emergency, call 911 or return to the ER.

What should I have ready?

Your discharge paperwork, current medication list or bottles, allergies, questions, and any home BP or glucose readings requested by your care team. Share medical records through the secure clinical intake link.

Can you refill my pain medication?

The clinician may consider appropriate non-controlled treatment after evaluation. Follow Up Dr. does not refill opioids/narcotics, benzodiazepines or stimulants.
