An insurance card alone does not confirm that a visit or treatment is covered. Ask about deductibles, copays, coinsurance, and any referral or authorization requirements.
Medical visits
Ask us to check your plan for the specific medical service and office before your appointment.
Procedures and aesthetic care
Some services are self-pay. Ask for consultation fees and an individual estimate before deciding.
Medication and treatment costs
A covered office visit does not necessarily mean a medication, laboratory test, or procedure is covered. For self-pay TRT and medical weight-loss visits, you pay AM&A for consultations and follow-up visits. Prescribed pharmacy medication is paid directly to the dispensing pharmacy and is not included in the visit fee. Ask separately about laboratory testing and any in-office treatment charges.
