What we can review after your ER visit

From a lingering symptom to a confusing scan report, we help you understand your ER visit and decide what should happen next.

For stable adults after a recent ER visit.

A practical bridge

You are 18 or older, have recently been discharged from the ER—generally within 14 days—and need outpatient follow-up. Follow the timing recommended in your discharge instructions, even if it is sooner than our 72-hour goal.

Start with telemedicine

Meet by video from a private place in California when clinically appropriate. We serve Orange County, the Inland Empire and Los Angeles. In-person visits are available in Mission Viejo, Corona, and Norwalk; procedures may require an office visit.

Unstable or having an emergency? Go back to the ER. For chest pain, severe trouble breathing, stroke symptoms, uncontrolled bleeding, or another medical emergency, call 911. Follow your ER return precautions. Do not wait for a follow-up visit or a reply to a request.

How to request your ER follow-up visit

Start with a brief request. Our team follows up by text or phone to confirm the right visit and help with the next steps.

Request your ER follow-up ↗
1

Request your visit

Have your discharge date, preferred visit format and insurance plan name ready. A request does not confirm an appointment.

2

Connect with our team

We review fit, availability and insurance or the fixed cash option. Then we help you complete secure booking and intake.

3

Share paperwork. Meet your clinician.

Please upload a photo or screenshot of your ER discharge paperwork through the secure intake link we provide, if available. Bring your questions and medication list. Missing paperwork is not an automatic barrier.

Use your insurance.

Is a visit within 72 hours guaranteed?

No. Our goal is a visit within 72 hours of your request, depending on availability and clinical suitability. Your ER discharge instructions may require care sooner. Call the team to confirm timing; seek urgent or emergency care when needed.

What if I do not have my ER paperwork?

Please upload discharge instructions, imaging reports and lab results through the secure intake link we provide if you have them. Missing paperwork is not an automatic barrier; our team can discuss what information is needed and how to request records.
