Turn instructions into next steps
Discuss the reason for referral, the appropriate specialty and the timeframe recommended by the ER. Referrals depend on your evaluation and insurer requirements.
Review unexpected findings and whether follow-up imaging, labs, specialist review or monitoring are clinically appropriate. Bring the report and any recommended timeline.
Discuss recovery and clinically appropriate short-term work notes at your visit. Notes are not backdated, and employer acceptance or specific restrictions cannot be guaranteed.
Keep your care connected
Keep that relationship. Follow Up Dr. provides a focused bridge after ER discharge. Our team can discuss sharing visit information with your ongoing clinician through the appropriate records process.
Ask about establishing primary care with Advanced Medical & Aesthetics. That is a separate ongoing-care relationship after your short-term transition needs are addressed.
Transition care is available for up to 14 days when appropriate. Referrals are not confirmed specialist appointments. Authorization, network participation and specialist capacity can affect timing.
Primary care at AM&A ↗For referring teams
For a stable adult recently discharged from the ER, call our team to discuss suitability and availability. Have the discharge date, recommended timing, reason for follow-up and referring clinician’s contact ready.
Our team provides the appropriate secure channel for records transfer. Do not submit patient records through ordinary website chat or text messages.
Referral information ↗A simple way forward
Start with a brief request. Our team follows up by text or phone to confirm the right visit and help with the next steps.
Request your ER follow-up ↗Have your discharge date, preferred visit format and insurance plan name ready. A request does not confirm an appointment.
We review fit, availability and insurance or the fixed cash option. Then we help you complete secure booking and intake.
Please upload a photo or screenshot of your ER discharge paperwork through the secure intake link we provide, if available. Bring your questions and medication list. Missing paperwork is not an automatic barrier.
Common questions
No. We can consider clinically appropriate referrals and help clarify next steps. Specialist acceptance, scheduling, network rules and authorizations are outside the follow-up visit’s control.
It may. The team checks your plan requirements. Some plans require your assigned primary care clinician to authorize testing or specialist care.
Follow the ER instructions. Do not wait for our visit goal if the recommended assessment is sooner. Call 911 or return to the ER for a medical emergency.
